Implant overdenture FAQ
What is an implant overdenture?
An implant overdenture is a removable denture that attaches to dental implants with ball or locator-style attachments. It is more stable than a conventional denture but can still be taken out for cleaning.
How many implants does an overdenture need?
Two implants are a common choice for a lower overdenture, and upper overdentures usually need more, often four. The right number depends on bone, the opposing arch and the retention needed, so it is a clinical decision for each case.
What parts does an implant overdenture need?
Each implant gets an overdenture abutment, either a ball attachment or a Wise-Lock abutment. The denture carries a housing with a replaceable retention cap for each abutment. For the lab you also need impression copings, analogs and laboratory processing caps.
What is the difference between ball and locator-style overdentures?
A ball attachment uses a ball-shaped abutment and an elastic cap that snaps over it. A locator-style attachment, such as Surcam's Wise-Lock, uses a lower-profile stud abutment and a replaceable cap, and Wise-Lock comes angled up to 45° for divergent implants.
Ball attachment or Wise-Lock: which should I choose?
Ball attachments are simple and familiar, with offset versions for implants that aren't parallel. Wise-Lock is lower profile, comes in angled versions up to 45° for divergent implants, and uses a separate prosthetic screw to limit loosening. Many clinicians choose by the space available and the implant angles.
Can retention caps be replaced chairside?
Yes. Caps sit in a metal housing in the denture and can be changed when they wear or when the patient needs more or less retention. Surcam sells replacement caps and housings in packs of 10.
Which implants do Surcam overdenture parts fit?
Surcam overdenture attachments have an internal hex connection. Ball attachments come for the 3.75 and 4.5 platforms, and Wise-Lock for the 3.75 platform.
What is a locator-style overdenture attachment?
A locator-style attachment is a low-profile stud abutment screwed into the implant, with a replaceable retention cap held in a housing in the denture. Wise-Lock is Surcam's own locator-style attachment, developed in-house, with its own caps, housings, impression copings, analogs and tools. Use Wise-Lock caps and housings with Wise-Lock abutments.
How do I correct for implants that are not parallel?
For ball attachments, use the multi-position offset version, straight or 20° angled, which can be seated in 6 positions. For Wise-Lock, use an angled abutment at 9°, 18°, 30° or 45°.
Which caps control retention?
Ball attachment caps come in extra soft, soft and standard. Wise-Lock caps come in soft, standard and hard. Both systems also have a laboratory cap for processing the denture, which is not for patient use.
