A posterior segment rehabilitation case in compromised bone conditions requiring an immediate loading protocol. Bone quality presented as D3 with limited cortical engagement.

U-Type implants were selected to maximize primary stability through their deep, knife-thread design and tapered body configuration. The macro-geometry enabled effective bone condensation during insertion.

Implants achieved insertion torque values between 35–45 Ncm, allowing for immediate loading with a screw-retained provisional restoration. Occlusal forces were controlled during the initial healing phase.

No additional augmentation procedures were performed. Implant positioning and stability allowed for a simplified surgical and restorative workflow.

Follow-up evaluation demonstrated stable osseointegration, preserved crestal bone levels, and predictable prosthetic outcomes.

Outcome

All implants achieved sufficient primary stability for immediate loading without the need for staged treatment. Clinical and radiographic follow-up confirmed stable peri-implant conditions, preserved bone levels, and successful functional integration of the prosthetic restoration.

Clinical takeaway

U-Type implants can provide predictable primary stability in D3–D4 posterior maxillary bone when controlled under-preparation is used.
